Physicians Instructions
If you are a GIPAP registered physician, you are required to login with your username
and password on the upper right hand corner of the Home Page
in order to submit a GIPAP application on behalf of your patient. If you experience
difficulties, please email us at GIPAP@themaxfoundation.org.
